Zarówno perły Epsteina, jak i guzki Bohna to drobne torbiele należące do grupy łagodnych zmian w błonie śluzowej jamy ustnej. Nie bolą, nie przeszkadzają i nie powodują dyskomfortu. Mają charakter wrodzony i przejściowy. Nie wymagają leczenia i u dzieci znikają zazwyczaj w ciągu pierwszych tygodni po urodzeniu, choć zdarza się, że pozostają w jamie ustnej nieco dłużej – na kilka miesięcy. Później znikają samoistnie – bez śladu i blizn. W przeciwieństwie do nich, pleśniawki to efekt zakażenia grzybem drożdżakowatym Candida albicans – bolą, powodują dyskomfort, przeszkadzają i trzeba je leczyć.

Guzki Bohna – biało-szare zmiany w jamie ustnej

Guzki Bohna to torbiele nabłonkowe, prawdziwe dotyczące gruczołów ślinowych wytwarzających śluz. Często zlokalizowane są na policzkowych lub językowych częściach wyrostków zębodołowych a rzadko na podniebieniu. Tworzą się najczęściej na policzkowej powierzchni górnego wału dziąsłowego. Czasem można je zaobserwować także na dolnym wale dziąsłowym – ale wówczas występują również na powierzchni wału górnego. Przypadki, że guzki Bohna występują tylko na dolnym wale dziąsłowym, są pojedyncze[1]. Więcej

Ozonowana oliwa z oliwek w przyszłości zastąpi tradycyjne profesjonalne bakteriobójcze płyny do płukania jamy ustnej? Tak, to się już dzieje, a bakterie próchnicy giną dzięki płynom z oliwą wzbogaconą ozonem.

Grupa badaczy z kilku włoskich uniwersytetów postanowiła sprawdzić in vitro, czy ozonowana oliwa z oliwek może być skutecznym środkiem przeciwko Streptococcus mutans. W tym celu przetestowano dwa różne płyny do płukania jamy ustnej, w składzie których znajdował się ten składnik. Jeden płyn zawierał ozonowaną oliwę z oliwek, a drugi oprócz niej zawierał również kwas hialuronowy i witaminę E. Żaden z płynów do płukania nie zawierał chlorheksydyny. Aktywność wszystkich próbek była analizowana w różnych rozcieńczeniach soli fizjologicznej (2-256 razy), tak aby odtworzyć rozcieńczenie śliny. „Królikiem doświadczalnym” były bakterie próchnicy z wyselekcjonowanego szczepu S. mutans.

Okazało się, że oba preparaty dobrze radziły sobie z inaktywowaniem bakterii próchnicy, a jednocześnie nie powodowały rozcieńczania śliny w jamie ustnej. Więcej

Statystyki dotyczące stanu zdrowia jamy ustnej dzieci są alarmujące: z badania Monitoring Stanu Zdrowia Jamy Ustnej z 2015 r. finansowanego przez Ministerstwo Zdrowia wynika, że co drugi 3-latek w Polsce ma zęby z próchnicą. Czasem na tyle zaawansowaną, że u tak małego dziecka  należy rozważyć ekstrakcję chorego zęba. To niedobrze, ponieważ zęby mleczne mają służyć do momentu fizjologicznej ich utraty, która związana jest z wyrzynaniem się zębów stałych. Te zaczynają wyłaniać się z dziąseł dopiero około 6. r. ż. Zakładając, że ekstrakcja zęba mlecznego nastąpiłaby w 3. r. ż. dziecka, do wyrośnięcia zęba stałego pozostają minimum 3 lata. To wystarczająco długi czas, aby przy niezastosowaniu leczenia protetycznego, dzieci, którym wcześnie usunięto chore mleczaki, zaczęły odczuwać pierwsze konsekwencje niewypełnienia luki po utraconym zębie.

Czy to oznacza, że zębów mlecznych nie należy zbyt szybko usuwać? Nic podobnego. Każdorazowo trzeba wykonać staranny bilans potencjalnych zysków i strat wynikających z wyboru ekstrakcji lub pozostawienia mleczaka w jamie ustnej. Bezwzględnym priorytetem powinno być jednak zdrowie zębów stałych i zdrowie ogólne. Jeśli więc ortodonta zaleci utrzymanie zęba mlecznego w szczęce najdłużej jak się da, natomiast dentysta orzeknie, że ten sam ząb należy usunąć, bo toczy go zgorzel i nie nadaje się on do leczenia zachowawczego, należy kierować się tutaj wskazaniami stomatologa i zdrowie dziecka przedłożyć nad estetykę uśmiechu. Jednocześnie nie należy rezygnować ze starań, aby dziecko piękny uśmiech jednak miało – co w praktyce oznacza, że po ekstrakcji mleczaka należy wypełnić powstałą po zębie lukę utrzymywaczem przestrzeni (tzw. protezką dziecięcą). Proteza powinna zabezpieczać przestrzeń w tym obszarze do czasu wyrośnięcia zęba stałego.

Jakie są wskazania do usunięcia zębów mlecznych?

• Zęby mleczne zdrowe – tu sytuacja jest jasna: powinny wypaść same lub – ze względu na wskazania ortodontyczne – powinny być usunięte wtedy, gdy zaleci to specjalista prowadzący leczenie. Usuwać należy także mleczaki zdrowe zaburzające wyrzynanie się zębów stałych.
• Zęby mleczne chore lub z wadami rozwojowymi – jeśli zęby mleczne są martwe, z torbielami lub chore, a w szczególności, jeśli są źródłem zakażeń tkanek miękkich i/lub twardych, toczy je zgorzel i nie ma możliwości leczenia zachowawczego, powinny być usunięte najszybciej, jak to możliwe – ponieważ ząb ze zgorzelą jest większym zagrożeniem dla zawiązka zęba stałego, niż ewentualne nieprawidłowości zgryzowe czy stłoczenia zębów. Stanowi też poważne zagrożenie dla zdrowia ogólnego dziecka.
• Wady rozwojowe twarzy lub jamy ustnej.
• Urazy zębów powodujące ich przemieszczenie, zwichnięcie lub złamanie – jeśli nie ma możliwości leczenia zachowawczego lub byłoby ono nieopłacalne.
• Zęby wrodzone i noworodkowe – kiedy nie ma możliwości zachowania ich w jamie ustnej niemowlęcia.
